What Your UHC Plan Covers Here

UHC members can receive the following at our practice:

For UHC members managing diabetes, that means your primary care, podiatry, lab, and wound care follow-up can stay coordinated in one clinic.

Verifying Your UHC Benefits

Pull out your UHC card and check the plan type printed on the front. Then follow the steps for your plan:

HMO and EPO members need a PCP on file. Pick a Moses Medical provider through uhc.com or by calling 1-800-328-5979. Your PCP writes referrals for specialist visits.

PPO members can book specialist appointments directly. No referral needed.

Medicare Advantage members should bring both their Medicare card and UHC MA card. Confirm Moses Medical is in-network for your specific plan before your visit.

Medicaid Community Plan members should bring their NY State Medicaid card and UHC Community Plan card. Our front desk verifies eligibility at check-in.

If any service needs prior authorization, our staff handles it.

Plan-Specific Tips

For employer-sponsored HMO/PPO/EPO members: Your plan covers a $0 annual wellness visit with preventive screenings, blood panels, and immunization updates. Schedule one every year with primary care.

For Medicare Advantage members: You qualify for an Annual Wellness Visit that includes a personalized prevention plan. Many MA plans also cover medical weight loss programs, hearing exams, and dental. Check your Evidence of Coverage or call UHC. What insurance plans do you accept?
We accept most insurance plans including Medicaid (MetroPlus, Fidelis, Amerigroup, Affinity, WellCare, Molina, UHC Community Plan, Hamaspik Choice), Medicare and Medicare Advantage plans, and commercial plans from Aetna, UnitedHealthcare, Cigna, Empire BCBS, EmblemHealth, Oxford, Oscar, and more. Do you accept Medicaid?
Yes, we accept all major New York Medicaid managed care plans including MetroPlus, Fidelis Care, Amerigroup, Affinity Health Plan, WellCare, Molina Healthcare, United Healthcare Community Plan, and Hamaspik Choice. Most Medicaid visits have zero copay.
What if I don't have insurance?
We see patients with or without insurance. We offer sliding scale self-pay visits starting at $50 based on your income. Our staff can also check if you qualify for Medicaid or help you enroll in a Marketplace plan. Visit our self-pay page at /self-pay for details, or call (646) 741-2111.
Do you accept Medicare?
Yes, we accept Original Medicare (Parts A and B) as well as Medicare Advantage plans from Aetna, UnitedHealthcare, Humana, Anthem, Empire BCBS, Cigna, Wellcare, and Fidelis Care. We participate in most Medicare supplemental plans as well.
Can you help me pick a health plan?
Absolutely.
