Our primary care team manages annual physicals, sick visits, diabetes, high blood pressure, asthma, cholesterol, medication questions, A1C checks, and preventive screening referrals such as mammograms and colonoscopies. If you are managing several conditions or medications, we review the full picture and help coordinate lab work, referrals, and follow-up.
