Here is what MVP members can get at Moses Medical:

- Podiatry for foot and ankle problems, orthotics, and diabetic foot care
- Lab services for blood draws, metabolic panels, and routine diagnostic tests, all on-site
Your providers and lab work can stay coordinated through the same clinic, with referrals handled when outside imaging is needed.
Confirming Your MVP Benefits
MVP offers HMO and PPO options, and the rules are different for each.
If you have an HMO plan, you need to choose a primary care provider. Specialist visits require a referral from that PCP. You can designate a Moses Medical provider as your primary care provider through the MVP member portal at mvphealthcare.com or by calling our office.
If you have a PPO plan, you can see specialists without a referral, but your copay may be higher for out-of-network visits. Staying in-network saves you money.
Not sure which plan type you have? Look at your MVP ID card or call MVP member services at 1-800-777-4793. You can also call us at (646) 741-2111 and we will check your eligibility before you come in.
Bring your MVP card and a photo ID to your first appointment. Our front desk verifies coverage and lets you know your copay before you see a provider.
Tips for MVP Members
Ask about generic medications. MVP maintains a formulary, which is a list of preferred drugs. When your provider writes a prescription, ask if there is a generic option on the MVP formulary. The price difference can be significant. Some generics cost $0 on MVP plans.
Use your preventive care benefits. Annual physicals, blood work, and age-appropriate screenings are covered at no extra cost. Many health problems surface in routine lab work before you notice any symptoms.
Keep specialist visits in-network. If you need cardiology or dermatology, getting those visits done at Moses Medical helps keep records and follow-up in one system.
Do not let your HMO referral expire. If your PCP writes a referral for a specialist visit, it typically has a time window. Book the appointment before it lapses so you do not need to go back for another referral.
