If you have an Affinity Health Plan card in your wallet, it works here. Moses Medical accepts Affinity Medicaid Managed Care and HMO plans at our office at 871B Westchester Ave in the Bronx. New York Medicaid managed care generally has no copay for most visits, though your cost share depends on your plan and the service.

Covered Services

Affinity members at Moses Medical can see primary care and several focused care options in one location. Here is what your card generally covers for Medicaid members:

Primary care for physicals, sick visits, immunizations, and chronic disease management. Internal medicine for ongoing blood pressure, cholesterol, diabetes, and chronic condition management. Dermatology for rashes, acne, eczema, and skin changes you want checked. Mental health visits for evaluation, medication questions or management when appropriate, and referral planning for therapy or counseling when needed.

Podiatry supports diabetic foot exams and routine foot care. People with diabetes in the Bronx need regular foot checks, which is why podiatry is one of the most important services on this list.

We handle lab work on-site and can help coordinate outside imaging when your provider orders it.

How to Use Your Affinity Card Here

Bring your Affinity Health Plan card and a photo ID. That is it. If you lost your card, call Affinity at 1-866-247-5678 for a replacement or to get your member ID over the phone.

Affinity requires you to pick a primary care provider. You can assign a Moses Medical provider by calling Affinity or by telling our front desk staff. The switch usually takes effect within a few days. If you currently have a different PCP and need care before the change processes, come in anyway. We will help you figure out next steps.

Affinity Medicaid may require a referral from your PCP before you see a specialist. Our primary care providers can write the referral during your visit. Your plan decides how quickly the specialist visit can be scheduled.

What Affinity Members Often Miss

Mental health support. Affinity members can ask about mental health visits at Moses Medical, including evaluation, medication questions or management when appropriate, and referral planning for therapy or counseling when needed. Call with your insurance card if you want help confirming plan details.

Diabetic foot exams. If you have diabetes, podiatry foot exams are generally covered under Medicaid managed care and are worth keeping up. Small cuts and blisters on your feet can turn into serious problems fast. Regular exams find them before they get worse.

Preventive screenings. Annual physicals, diabetes screening, blood pressure checks, cholesterol tests, and cancer screenings are generally covered with little or no cost share. These are the visits that keep you out of the emergency room. Ask Affinity what your plan covers and how often.
