Setting Up Your First Visit
- Find your benefit tier. The National Benefit Fund has different tiers based on your employer and job classification. Call the Fund at 1-646-473-9200 or check 1199seiubenefits.org to see which tier you are on.
- Bring your benefit card and a photo ID. We verify your coverage at the front desk.
- Ask about referrals. Some tiers require a referral from your primary care doctor before you see a specialist. If you need one, we can write it the same day.
- Know your copay. Copays depend on your tier and the type of visit. We will tell you the amount before your appointment starts.
