Our Partnership with Jacobi Medical Center
Moses Medical and NYC Health + Hospitals/Jacobi collaborate to serve patients in the eastern Bronx neighborhoods of Parkchester, West Farms, and East Tremont.
Trauma and Recovery
Jacobi Medical Center is a Level 1 Trauma Center. When patients are discharged after traumatic injuries, they need consistent follow-up care for pain management, physical therapy, and primary care monitoring. Moses Medical provides this continuity.
Shared Specialties
Our partnership focuses on primary care follow-up, pain management for post-surgical and post-trauma patients, and physical therapy referrals. Jacobi handles the acute care; we handle the long-term recovery and wellness.

If you’ve been discharged from Jacobi and need follow-up care, we’re accepting new patients with same-week availability. We speak English, Spanish, Bangla, and Hindi.
